If your child was placed in a class you need to secure the spot with an Enrollment Confirmation Form and Enrollment Fee. When the school offers you a spot, they will send an email confirming the offer with a link to the online Enrollment Confirmation Form. If you do not receive this email, please call the school.
Every spring, we send DCP families a set of enrollment forms that pave the way for the opening of the following school year. Please be considerate of the stated deadlines since all forms need to be processed and made available to teachers and staff. For your child's health and safety, certain forms are required to be on file in order for your student to attend class. As you proceed through the online pages, there will be a lot of information to process. Please take your time and be sure to read each screen page from top to bottom so you will be aware of what is expected of you and your family in the upcoming year.
The following TWO sets of secure forms must be completed by May 3 (or ASAP for late acceptance/late start) to secure your child's place in school for the upcoming school year. Each form will take at least 10 minutes to complete. Please make sure you have all of your personal, medical and emergency information available.
Additionally, the Child's General Health Appraisal Form MUST be printed and it, along with a Proof of Immunization Card (or Waiver, if applicable), must be filled out by your child’s doctor and returned to the school by August 16, 2010. Please note that we have not included an immunization form with this registration packet. Most doctors have their own forms that they fill out. If this is not the case and you need an immunization form, please let us know and we will be happy to supply you with one. A pdf copy of the Child's Statement of Health Status will be available in the online form and in your confirmation email. Please print it out and bring it to your child's doctor.
You will need your MyDCP account information in order to complete the enrollment forms. If you don't have a MyDCP Acoount or are unsure, please contact thriva@dcpplay.org. Your MyDCP Online Account uses the same email address and password you used to complete the Lottery, Waitlist and/or Enrollment Confirmation forms online over the past few months.
